As part of our application, clients applying for the Fresh Start Mortgage Relief Program or the National Mortgage Relief Program are required to fill out and complete:
1. Property Fact Form
2. Sign and notarize an authorization form that will be used to communicate with your lender, servicer or mortgage company.
